NUVO, has to have a signed waiver form from each person attending NUVO.  Please to to the site: www.gonuvo.com and click on registration, then the second page of the registration is the waiver form.  PRINT ONLY THE WAIVER FORM fill out and turn into Liz in the office (there will be a folder on her desk for the foms / YOU MUST HAVE A FORM to participate)
